The practice provides personal medical services to patients who live within an agreed geographical area determined by the practice. If you live within our area, you can ask to be registered with the practice and, providing the practice list is not closed, you may be registered as a patient with the practice.

To register, either bring your medical card to the practice or, if you are not able to find this card, we can provide a form for this purpose. New patients are asked to complete a lifestyle questionnaire when applying to join our list. Babies will be provided with a form FP58 from the registrars office, and this will take the place of a medical card for registration purposes.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
