If you are travelling to a country within the European Economic Community (EEC), which includes the European Union you are strongly advised to obtain a European Health Insurance Card (EHIC). This is a simple process.
This is in addition to any travel insurance you may already have taken out.
Most travel insurance policies require travellers to be in possession of a valid EHIC.
For full details both about it and how to apply please click here.
Even if you already have an EHIC please ensure it is valid for your entire stay.
Travel Vaccination Service
Start preparing for your trip four to six weeks before you go. As a surgery we provide a private clinic for travel vaccinations. Travel Clinic appointments are a private service and not part of the NHS, the appointments with our Nursing team are free of charge. Haresfield Surgery do however charge an administrative fee of £20.00 for processing travel forms and to cover the cost of the extra administration time and nurse’s time required to run the clinic.
Please print and complete both forms:-
Form 1 is to be completed once for the trip Click here to download form 1 – 1 per trip
Form 2 is to be completed by every patient going on the trip and will be used during your travel clinic appointment. Click here to download form 2 – 1 per patient
Forms will not be processed and appointments will not be scheduled unless we receive the administration fee.
All vaccinations necessary for travel to other countries are given by our nurses. The practice is also a recognised centre for administration of Yellow Fever Immunisation.
